Your membership dues entitle you to many benefits which help protect you and your family. On this page you'll find the resources you need to get the most out of those benefits and keep your membership current.
Benefits Changes - 2010-2011
There will be an updated benefits guided posted here soon, and details will be announced in our upcoming newsletter, Firefighter 411! In the meantime, changes to your benefits for the upcoming year appear below.
NEW! The Relief 5-15: Active duty members are covered by The Relief 5-15 Plan, a self-funded benefit similar to Short-Term Disability which protects your income in case of disabling injury or illness. You will be covered for up to 6 months and benefits are payable beginning only 2 weeks after your disability begins. This plan will provide more benefit for our injured and ill members than the CSFA LTD benefit it replaces. More details will be released soon.
NEW! All SDFRA members are now members of the San Diego Memorial Society: The San Diego Memorial Society is a local non-profit organization which protects consumers and families by securing fixed and reduced costs for a variety of funerary services. Spare your family difficult and painful decisions at this sensitive time by making pre-need arrangements, or be assured that your family won't be taken advantage of at the time of need. More details available soon.
CSFA*: SDFRA no longer automatically pays the CSFA dues for all of our active duty and retired members. However you can still be reimbursed for the cost of CSFA membership through the FlexFund. Simply turn your receipt into us to submit it for reimbursement.
*Please note- active duty members are no longer automatically covered by the CSFA Long-Term Disability plan. This has been replaced by the Relief 5-15 Plan. Watch for our newsletter for more details.
Membership Forms
Member Enrollment Form - if you need to update your address, phone, spouse/dependents, or other important information
Beneficiary Form - Active - to change your information, or information about your beneficiaries
Payroll Deduction Form - Active - to authorize payment of your membership dues through payroll deduction
Payroll Deduction Form - Retired - to authorize payment of your membership dues through payroll deduction
